The easiest ways to access your GP health record are:
- through your NHS account (via the NHS App or NHS website)
- using the practice’s online system: SystmOnline
To access your record online, you must be:
- registered with the surgery
- aged 16 or over
Getting help with your GP health record
Contact the surgery if:
- you need a printed copy or a digital version of your record
- you cannot see your record online
- information is missing or has disappeared
- you can only see recent information and need older records
- something in your record looks incorrect
- you can see information that should not be there
- you do not want online access to your record
- someone is pressuring you to show them your record (we can remove access for your safety)
What’s in your GP health record?
Your GP health record includes information such as:
- medicines your GP has prescribed
- allergies
- vaccinations
- conditions you have been diagnosed with
- notes from GP appointments
- results of tests the surgery has arranged
- letters from specialists or hospitals that have been sent to your GP
Your GP record does not include information held only by other services (such as hospitals, dentists, opticians or private providers).
To access these records, you will need to contact those services directly.
