How to Request an Appointment
You will need to complete a short online form, which allows our triage team to assess your needs and direct you to the most appropriate care.
1. Online Form (Fastest & Preferred Method)
- Go to SystmConnect
- Select the appropriate form (e.g., urgent medical appointment, review appointment, vaccination appointment)
- Provide details about your health concern, including relevant photos if applicable
2. NHS App or SystmOnline
- Log in to the NHS App or SystmOnline to complete the Total Triage form
- Scroll down if the form is not immediately visible
3. Telephone Assistance
- Press 1: Receive a link via text to complete the form on your phone
- Press 3: Speak to a care navigator if you need help completing a form
4. Walk-in Assistance
- Our care navigators can help you complete the form if you visit the surgery in person
Nurse & HCA Appointments – No Form Needed
You do not need to complete an online form to book certain appointments with our Nurses or Healthcare Assistants (HCAs). These can still be booked by calling the surgery directly.
This includes:
- Cervical smears
- Wound care/dressings
- Blood pressure checks
- B12 injections
- NHS health checks
- Stitch removal
- Annual reviews (e.g., asthma, diabetes, hypertension)
- Other routine nurse or HCA procedures
If you’re unsure, just give us a call and our Care Navigators will advise you.
What Happens After You Submit a Form?
- The Triage Team will review all requests on the same day
- You will receive a text or phone call within 36 working hours, based on urgency
- If urgent, you will be contacted the same day to arrange an appointment
- If no urgent slots are available, you will be advised to contact NHS 111
- Some queries may be resolved via text, email, or referral to another service

Other Ways to Access Care
- Pharmacy Advice: Pharmacists can help with common conditions like coughs, colds, flu, skin rashes, aches, and pains
- Urgent Treatment Centres: For urgent but non-life-threatening issues, visit your nearest centre
- Self-Care at Home: Many minor illnesses can be managed with rest and over-the-counter medication
- NHS 111: For non-emergency medical advice
Home Visits
- Reserved for housebound patients only
- Request before 11 AM by using the SystmConnect service
- Home visits will not be granted due to lack of transport, childcare issues, poor weather, or financial constraints
Extended Hours & Weekend Appointments
- Available Monday to Friday from 6:30 PM
- Weekend appointments available – ask our care navigators to book
- All appointments must be pre-booked

- A Care Navigator is available to guide patients over the phone with basic queries, such as where to find the correct link.
- For more in-depth, one-to-one digital support, our Social Prescriber, Phil, is available every Wednesday and can offer up to 30 minutes of guidance.
