Accessing Someone Else’s Information

Parents, family members or carers can sometimes access services on someone else’s behalf.
This is called proxy access.

We can set this up if both you and the person you care for are registered with us.

How to request proxy access

To request access:

  1. Collect a Proxy Access Form from reception (see our opening times)
  2. Provide proof of your identity
  3. Provide proof that you have permission (e.g. signed consent, parental responsibility, or official documentation)
  4. Our team will review and set up access if appropriate

We may need to speak with the patient directly (where appropriate) to confirm consent.

Linked profiles in your NHS account

Once proxy access is set up, you can:

  • view the other person’s linked profile
  • order prescriptions for them
  • view parts of their GP record
  • manage appointments
  • access information through the NHS App or NHS website

Safeguarding and restrictions

We may decline or limit proxy access if:

  • giving access could put the patient at risk
  • the patient declines consent
  • the patient is aged 13–15 (special rules apply)
  • there are concerns about coercion or privacy

We will always follow national safeguarding guidance.
