Upload Photos or Documents

Patients may sometimes be asked by a clinician to send photographs or documents to help with their care.

Please only upload photos or documents if you have been asked to do so by a member of our clinical team.

Uploaded images or documents will be sent securely to the practice and added to your medical record where appropriate.

Important information

  • Images you send form part of your confidential medical record
  • They will be reviewed by a clinician at the practice
  • They may be shared with other healthcare professionals involved in your care if necessary
  • Please ensure photographs are clear and in focus

Tips for taking helpful photos

If you are sending a photograph of a skin problem or other condition, please try to:

  • Use a phone or camera with good image quality
  • Take photos in good lighting
  • Take more than one photo
  • Include one photo from further away to show the location on the body
  • Take a close-up image of the area
  • Keep the affected area in the centre of the image
  • Avoid blurry or out-of-focus images

Please do not send photos unless requested by a clinician.
