Community staff are aligned to our practice. We work as a team with the district nurses and various other specialists to provide the appropriate health and social care.

Patients or their relatives may contact the members of the team directly, or the patient may be referred by another member of the healthcare team. Patients may be seen at home or at the surgery by the community staff.
