Care Navigators

Helping you access the right care, at the right time.

Our Care Navigators are trained members of the practice team who help patients access the right healthcare professional or service for their needs.

They play an important role in ensuring patients receive the right care, at the right time, by helping them navigate the range of services available both within the practice and across the NHS.

When you contact the practice, a Care Navigator may ask for a brief description of your problem or enquiry. This allows them to direct you to the most appropriate clinician or service, whether that’s a GP, nurse, clinical pharmacist, physiotherapist, healthcare assistant or another NHS or community service.

Care Navigators are trained in confidentiality and handle your information with the same strict level of confidentiality as every other member of the practice team. They do not diagnose medical conditions or provide clinical advice.

By helping patients access the most appropriate service first, Care Navigators improve access to healthcare and ensure GP appointments remain available for those who need them most.
