Changing your contact details

It’s important to keep your contact details up to date so we can reach you about appointments, test results and your care.

You can update your details in any of the following ways:

Change of address

If you have moved home, please check whether you are still in our catchment area.
You must live within this area to stay registered with the practice.

If you are still within the boundary, you can update your address online or via our admin form.

Changing your phone number or email address

We may need to see proof of identity before updating your phone number or email address, particularly if the change is made on your behalf or if we cannot verify your identity online.

Changing your name

To change your name on our records, we will need to see official proof of the change.
Accepted documents include:

  • marriage or civil partnership certificate
  • deed poll
  • birth certificate

You can submit this via our online admin form or bring the document to reception.

Frequently Asked Question

Do I need to come into the surgery to update my details?
Not always.

You can update most details through your NHS App, NHS account, SystmOnline, or by completing an admin request form.

For changes that require proof (such as a name change), you may need to upload a document or bring it to reception.

What if I have moved outside the catchment area?
If your new address is outside our boundary, you may need to register with a GP closer to your new home.

We will let you know if this applies once you submit your address update.

Can someone else update my details for me?
Someone can help you complete the form, but we may need to verify your identity before making changes — especially for phone numbers, email addresses or name changes.

How long will it take for my details to be updated?
Most updates are completed within a few working days.

Name changes or changes requiring verification may take slightly longer.

Can I update my details over the phone?
For security reasons, we may not be able to make certain changes over the phone.
You may be asked to complete an online form or provide proof of identity.

Do I need to inform anyone else of my new details?
Updating your GP record does not automatically update your details with hospitals, your employer, or other services.

You may need to inform them separately.
