PLEASE READ BEFORE SENDING A REQUEST.

  1. Please fill out the form with accurate details and give as much information as you can about your symptoms or request.
  2. Please state clearly if you prefer a face-to-face or telephone appointment. If you do not specify, our Care Navigators will book the most appropriate option for you.
  3. If you are filling out the form on behalf of someone else, you must use that person’s details (not your own). This is because the information links directly to the patient’s medical record.
  4. If forms are filled out incorrectly, or without enough detail, you will be asked to complete the form again.
  5. Forms open at 8:00am each day (Monday – Friday) and close once we reach capacity

How to Request an Appointment

We use a system called SystmConnect. You’ll need to fill out a short online form so our triage team can understand your needs and direct you to the right care.

1. Online Form (Fastest & Preferred)

  • Go to SystmConnect.
  • Choose the correct form (e.g. New Condition, Follow-Up, Other Medical Request etc).
  • Provide clear details and upload photos if helpful.

2. NHS App or SystmOnline

  • Log in to the NHS App or SystmOnline.
  • Look for the Total Triage form (scroll down if you can’t see it straight away).

3. By Telephone

  • Press 1: receive a text link to complete the form on your phone.
  • Press 3: speak to a Care Navigator if you need help filling in the form.

4. Walk-in Support

Our Care Navigators can help you complete the form if you come into the surgery.

Nurse & HCA Appointments – No Form Needed

Some appointments can still be booked directly by calling us, such as:

  • Cervical smears
  • Wound care/dressings
  • Blood pressure checks
  • B12 injections
  • NHS health checks
  • Stitch removal
  • Annual reviews (e.g. asthma, diabetes, hypertension)
  • Other routine nurse or HCA procedures

If you’re unsure, call and our Care Navigators will guide you.

What Happens After You Submit a Form?

  • The triage team reviews all forms the same day.
  • You’ll get a text or call within 36 working hours, depending on urgency.
  • Urgent cases are contacted the same day. If no urgent slots are left, you’ll be advised to contact NHS 111.
  • Some queries may be sorted by text, email, or referral to another service.

Other Ways to Access Care

  • Pharmacy Advice: for coughs, colds, flu, skin rashes, aches, pains.
  • Urgent Treatment Centres: for urgent but non-emergency issues.
  • Self-care: many minor illnesses improve with rest and over-the-counter medication.
  • NHS 111: for medical advice when the surgery is closed.

Home Visits

  • Reserved for housebound patients only.
  • Please request before 11am through SystmConnect.
  • We cannot offer home visits for transport problems, childcare issues, poor weather, or financial reasons.

Extended Hours & Weekend Appointments

  • Available Monday to Friday from 6:30pm.
  • Weekend appointments also available.
  • All must be pre-booked – speak to a Care Navigator to arrange.

Need Help With Online Services?

We understand online systems can feel new or tricky. Support is available:

  • A Care Navigator can guide you over the phone with simple queries.
  • For one-to-one support, our Social Prescriber, Annabel, is available every Wednesday for up to 30 minutes of digital help. Contact us to arrange this.
Frequently Asked Questions (FAQ)

What if my problem is urgent?

If life-threatening, call 999.
For urgent GP issues, select “urgent medical appointment” on the form.
If no slots are available, call NHS 111.

2. Can I still request appointments by phone?

You cannot request a GP appointment over the phone.
Calling 01767 312 441 will direct you to complete a SystmConnect form online.
However, you can still book nurse and HCA appointments by phone.
If you need assistance, our care navigators can complete a form for you.

3. What if I don’t have internet access?

A relative, carer or friend can fill out the form for you.
If no one can help, call the surgery, and our team will assist.

4. How quickly will I receive a response?

  • Urgent requests: Handled same day
  • Routine requests: Typically responded to within 2 working days

5. Can I walk in to book an appointment?

Walk-ins are not recommended.
If you visit reception, you will be asked to fill out a SystmConnect online form and wait for a response.

6. What if the online system says “full” or is paused?

If you cannot complete a SystmConnect online form, it means we have reached capacity.
Call NHS 111 if your issue cannot wait.
