Please only upload photos or documents if you have been asked to do so by a member of our clinical team.
Uploaded images or documents will be sent securely to the practice and added to your medical record where appropriate.

- Images you send form part of your confidential medical record
- They will be reviewed by a clinician at the practice
- They may be shared with other healthcare professionals involved in your care if necessary
- Please ensure photographs are clear and in focus
Tips for taking helpful photos
If you are sending a photograph of a skin problem or other condition, please try to:
- Use a phone or camera with good image quality
- Take photos in good lighting
- Take more than one photo
- Include one photo from further away to show the location on the body
- Take a close-up image of the area
- Keep the affected area in the centre of the image
- Avoid blurry or out-of-focus images
Please do not send photos unless requested by a clinician.
