Community Staff
Our practice is supported by a dedicated team of community staff, including district nurses and various other specialists, to provide integrated health and social care.
Patients or their relatives can contact the team directly, or referrals can be made by a healthcare professional. Depending on individual needs, patients may be seen either at home or at the surgery by the community staff.
Mental Health Link Worker
We have a mental health link worker who works alongside the team, providing assessment and support for our patients. They help individuals manage their mental health by offering guidance, connecting them to appropriate services, and supporting their overall wellbeing.

Social Prescriber
Our Social Prescriber provides 30-minute appointments to support patients with non-medical needs that may be affecting their health and wellbeing. She helps identify underlying issues such as mental health concerns, financial difficulties, or social care needs and connects patients with appropriate community resources and services.

First Contact Physiotherapists
Amtul Mateen
Amtul is a fully qualified Musculoskeletal Physiotherapist with over six years of experience. She specialises in musculoskeletal health, offering expert assessment, diagnosis, and management of joint, muscle, and soft-tissue conditions.
She has completed a Master’s in Orthopaedics and Rehabilitation and works to deliver high-quality, person-centred care.
Gayathri Mitham
Gayathri is part of our experienced First Contact Physiotherapy team, providing assessment and support for patients with musculoskeletal concerns.
FCPs provide expert assessment for musculoskeletal problems without needing a GP referral.
