Alongside our practice team, we work with a range of healthcare professionals and community services to provide additional care and support.
Depending on your needs, you may be offered an appointment with one of these teams instead of, or alongside, a GP or nurse.
Community Nursing Team
Our practice is supported by community healthcare professionals, including District Nurses, who provide care for patients who need nursing support in the community.
Depending on your needs, care may be provided at home, in a community setting or at the surgery.
Some community services can be contacted directly, while others require a referral from a healthcare professional.
Mental Health Link Worker
Our Mental Health Link Worker works alongside the practice team to provide support to patients experiencing mental health difficulties.
They can help assess your needs, provide guidance and connect you with appropriate mental health or community services where needed.
Clinical Care Coordinator
