Community Staff
Our practice is supported by a dedicated team of community staff, including district nurses and various other specialists, to provide integrated health and social care.
Patients or their relatives can contact the team directly, or referrals can be made by a healthcare professional. Depending on individual needs, patients may be seen either at home or at the surgery by the community staff.
First Contact Physiotherapist
Starting from mid-April 2025, we will have a First Contact Physiotherapist (FCP) available for patients with musculoskeletal conditions. They provide expert assessment, advice, and management plans without the need for a GP referral, helping patients with issues such as joint pain, back pain, and soft tissue injuries.
Mental Health Link Worker
We have a mental health link worker who works alongside the team, providing assessment and support for our patients. They help individuals manage their mental health by offering guidance, connecting them to appropriate services, and supporting their overall well-being.
Social Prescriber
Our Social Prescriber provides 30-minute appointments to support patients with non-medical needs that may be affecting their health and well-being. They help identify underlying issues such as mental health concerns, financial difficulties, or social care needs and connect patients with appropriate community resources and services.
Clinical Pharmacists
We have clinical pharmacists working alongside the team to support medication management. They assist with medication reviews, prescribing, and addressing any queries related to prescriptions to ensure safe and effective treatment for our patients.
